Back-to-School Forms for 2008-2009

All forms are not needed by every student and some forms have space for more than one child. Info for each form is listed below along with a link to download the forms in PDF.

• Emergency Treatment Form--One form needed for each student.
  
  
• Student Health Update--One form needed for each student.
  
  
• Ethical Use of the Network--For students in grades 3-12.
  
  
• Parent Pledge--For students in grades 6-12.
  
  
• Athletic Release--For students in grades 7-12 who will be playing on school teams.
  
  
• School/Class Trip Authorization--One form needed for each student.
  
  
• Lower School Carpool Information--For all lower school students.
  
  
• Lower School & Key School Before- and After-School Program--Needed only if your child arrives before 8 A.M. or needs to stay after 3 P.M.

• Student Health Update--Must be completed each year for each student.
  
• Student's Health Report--Must be completed by a physician for all new students and annually for students in PK, kindergarten, first grade, fourth grade, seventh grade, and tenth grade.
